Each unit is a single-use device. Most patients use one per day, so a 30-day supply is 30 units.
Automatic delivery every 30 days. Pause or cancel any time, with no fee. Most popular.
A single month's supply with no recurring commitment.
A one-week trial to confirm fit and comfort before committing to a full supply.
This estimates what you may be able to claim back after purchase. It does not reduce what you pay today.
Monthly Subscription · One size fits all
Submitting this form does not complete a purchase. A member of our team will contact you to confirm the order and take payment. Reimbursement figures are estimates only and are not a guarantee of payment. What you actually recover depends on your plan, your deductible status, the Medicare approved amount in your area, and physician documentation of medical necessity. Acosta Medical Group cannot guarantee that any claim you file will be paid.
One-size design
Most external catheters require you to measure, pick from four or five sizes, and hope you chose correctly. Uro-Gard does not. The system is engineered as a single universal size that accommodates the full range of adult male anatomy.
That removes the most common reason external catheters fail in practice: the wrong size was ordered, so it leaked or it was uncomfortable, so the patient stopped using it.
Order your first supplyNo tape measure, no fitting appointment, and no clinician visit required simply to place an order.
Nothing to exchange and no reorder delay because the size you picked turned out not to fit.
One product, one item number. Refills and subscriptions never require re-selecting anything.
Family members and facility staff can order confidently without knowing measurements.
How payment and reimbursement work
Being clear about this up front matters more to us than making the checkout look cheaper than it is.
We are not enrolled as a Medicare DMEPOS supplier. We do not bill Medicare, Medicare Advantage, VA, or private insurance on your behalf. Every order placed on this site is an out-of-pocket purchase paid in full at checkout.
If you are entitled to reimbursement, you file the claim with your plan yourself, and we give you everything you need to do it.
You pay the full price at checkout by card. Nothing is contingent on insurance, so there is no eligibility check, no prior authorization wait, and no surprise balance bill later.
Every order ships with an itemized receipt showing the HCPCS code (A4326), quantity, date of service, and our supplier information, formatted for a reimbursement claim.
You submit that receipt to your plan with your physician's order and documentation of medical necessity. Reimbursement, if approved, is paid directly to you.
Uro-Gard carries HCPCS code A4326, a male external urinary collection device under the Part B durable medical equipment benefit. For eligible patients, Medicare may reimburse up to 80% of the Medicare-approved amount after the annual Part B deductible is met.
The Medicare-approved amount is set by fee schedule and varies by region. It is not the same as our retail price, so your actual reimbursement may be lower than the estimate shown at checkout.
Medicare Advantage plans administer the same benefit under their own rules and commonly require prior authorization before they will reimburse. Many commercial plans cover external catheters as durable medical equipment. Veterans may have separate entitlement through the VA.
In every case the claim is yours to file. Check with your plan before purchasing if reimbursement is important to your decision.
Most plans require a written order and documentation of medical necessity in your medical record before they will reimburse an external catheter. We can send a documentation request template to your physician's office on request, but the order itself has to come from your clinician.

Questions
No. Uro-Gard is entirely external. There is no urethral insertion of any kind, which is the main reason external catheters avoid the trauma and biofilm risk associated with indwelling (Foley) catheters.
Absorbent products hold urine against the skin. Uro-Gard directs urine away from the body into a contained pouch and then to a drainage bag, so skin contact is minimal. It is also billable to Medicare under HCPCS A4326, which absorbent products generally are not.
Most patients use one unit per day, which is why a 30-day supply is 30 units. Your clinician may recommend a different frequency based on your situation.
Yes. The system is designed to be worn discreetly under everyday clothing, and the collection bag detaches for mobility and activity.
No. Uro-Gard is one size fits all. The system is designed as a single universal size that accommodates the full range of adult male anatomy, so there is nothing to measure and no size to choose. If you would like to try it before ordering a full supply, request a free sample or start with the seven-unit Starter Pack.
No. Acosta Medical Group is not enrolled as a Medicare DMEPOS supplier and does not submit claims to Medicare, Medicare Advantage, VA, or private insurance. Every order is an out-of-pocket purchase paid in full at checkout. If you are entitled to reimbursement, you file that claim with your plan yourself, and we provide an itemized receipt with the HCPCS code and all details a claim requires.
Not to purchase. You can buy Uro-Gard out of pocket without one. However, most plans require a physician's written order and documentation of medical necessity before they will reimburse you, so if reimbursement matters to you, speak with your clinician before ordering.
We cannot promise a figure. Medicare Part B may reimburse up to 80% of the Medicare-approved amount for eligible patients, but that approved amount is set by fee schedule, varies by region, and is not the same as our retail price. The estimate shown at checkout is illustrative only. Check with your plan if the amount is material to your decision.
Yes. Shipments arrive in plain packaging with no product branding, imagery, or description on the outside.
Support

Family caregivers order on behalf of a loved one every day. We can ship to a different address than the billing address and send the documentation request to the patient's physician. Payment is still out of pocket at checkout.
If your loved one is in an assisted living or memory care community, ask us about coordinating supply with the facility.
